Rural Northwest Health provides residential services for community members at the Hopetoun and Warracknabeal Campuses. We provide services for permanent placement respite and for people living with dementia for people assessed as having high and low care level of care needs.The Board of Management has set the strategic goal of meeting the community needs for aged care services from 2011-2015. Rural Northwest Health’s vision for our residential and community aged care services is that we will provide a service to the people we are privileged to work with that is Person centred and focuses on what and how and when the residents would like to have things done.Capability focused so that the resident has the continued capacity and the opportunity to continue to do the things that they want to do. Focused on living life to the full and allows the resident to continue to share their knowledge and skills and learn new knowledge and skills. A partnership with families friends stakeholders and partners that demonstrates best practice projects in social physical emotional psychological and spiritual wellness. Safe secure and welcoming and provides access to both indoor and outdoor activities and a range of public and private spaces for resident’s families and friends to enjoy.
